Students, families, and staff of the Howard-Suamico School District are invited to receive the Pfizer COVID-19 vaccine in collaboration with Prevea Health.

Students, families, and staff of the Howard-Suamico School District are invited to receive the Pfizer COVID-19 vaccine in collaboration with Prevea Health. You do not need to be a Prevea patient to receive a COVID-19 vaccine at the Bay Port vaccination clinic.

The COVID-19 Vaccination Clinic with Prevea Health will be located at Bay Port High School, 2710 Lineville Road, Green Bay. Please use the main entrance doors and proceed to registration.

First dose of Pfizer - Saturday, June 5, 7 a.m. to 4 p.m.
Second dose of Pfizer - Saturday, June 26, 7 a.m. to 4 p.m.

Please note

  • Individuals who are 12 to 15 years of age must have a parent/guardian present for both doses.
  • Individuals who are 16 or 17 years of age are required to have parent/guardian consent to receive vaccination. In the event that a parent/guardian cannot be present, a consent form must be signed and brought to both doses.
  • There is no cost for the vaccine and immigration status is not needed. Translation device will be available.
